Adopt
Adopt through Forever Care
This program is all about placing dogs into safe homes before shelters have the opportunity to euthanize them.
What you’ll need
To adopt a dog, Forever Care needs you to submit an application along with documents to verify your income, such as:
- Valid driver’s license or picture ID
- Proof of fixed income (retirement check, public assistance or food stamp evidence)
- A recent utility bill at least 3 months old (if that’s an issue, explanations are accepted)
- Photos of your home — inside, front, side and back (the more the better)
- Copy of your lease, deed, proof of ownership or rental agreement
- Cell phone number (verified when you pick up your dog)
- Contact info and cell numbers for 3 friends or relatives
You can upload these right on the application — or bring them in later.

Don’t qualify for assistance? You can still adopt.
Dogs featured through this program are available to everyone — individuals, businesses, hospitals, retirement homes, schools and more. Fill out the same application and choose “no assistance needed.”
